About this role

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide comprehensive care for clients at their homes in Palm Beach Gardens. The ideal candidate will have experience in a variety of nursing duties and be comfortable providing total care to an individual with complex medical needs. Shifts are 8 pm to 8 am or 8 am to 8 pm per diem.

Deliver exceptional care to the dedicated client following established care plans and physician orders that can be safely provided in the home. Administer medications and treatments as prescribed, monitor vital signs, and report any changes to healthcare professionals. Educate patients and families on proper healthcare procedures while maintaining accurate patient records.

Requirements

  • Active LPN license in Florida
  • BLS Certification
  • A minimum of 2 years related experience
  • Proven experience providing nursing care, including tube feeding, straight cath, and the use of a Hoyer lift
  • Comfortable with total care responsibilities and maintaining the dignity and privacy of the client
  • Strong communication skills and the ability to follow care plans accurately
  • Must be able to lift and transfer up to 180 lbs with assistance of Hoyer lift and other assistive devices
  • Ability to drive a transport van, with a valid driver’s license and clean driving record

Responsibilities

  • Deliver exceptional care to the dedicated client
  • Provide direct patient care in accordance with established care plans and physician orders
  • Administer medications and treatments as prescribed by healthcare providers
  • Monitor patient vital signs and condition, and report any changes
  • Educate patients and their families on proper healthcare procedures and techniques
  • Maintain accurate and up-to-date patient records and documentation
  • Perform tube feeding, straight cath, and use of a Hoyer lift
