Home Visit Registered Nurse (RN)

We are hiring a PRN Home Visit Registered Nurse. The Registered Nurse provides and directs provisions of nursing and personal care to patients in their homes as prescribed by the physician or authorized by the state. Coordinates the plan of care with physician, agency personnel, patient, family, case manager, and community resources.

Provides high quality clinical services within the scope of practice and infection control standards, in accordance with the plan of care. Completes nursing assessments per federal and state requirements and ensures patient eligibility and medical necessity. Develops and revises individualized plans of care with other community providers.

Performs treatments and interventions according to proper technique and administers medications per physician order. Ensures accurate and timely visit documentation and reports clinical findings to clinical manager and physician. Participates in patient case conferences and RN on-call rotation if required.

Completes all orientation and annual training timely as required by the organization. Participates in the organization’s Performance Improvement and Compliance programs. Assists with the orientation of agency staff as needed.
