As an Advanced Practice Clinician, provide comprehensive primary care services including routine rounding and proactive chronic disease management. Coordinate care across interdisciplinary teams and collaborate closely with facility partners. Emphasize preventive care to improve quality outcomes and experiences for patients, families, and care teams.
Partner with physicians, behavioral health specialists, care coordinators, and facility staff to create integrated, person-centered care plans. Requirements
- Current NP or PA licensure in the state of practice
- Previous professional patient care experience
- Experience in geriatrics, family medicine, or internal medicine preferred
- Exceptional patient-centered service skills including compassionate communication and active listening
- Professionalism and ability to build trusting relationships with residents, families, and facility staff
- Strong collaboration skills to work effectively across interdisciplinary teams
