The Visit Registered Nurse assesses and identifies patient/family needs, and plans, implements, evaluates and documents individualized hospice nursing care in accordance with the interdisciplinary plan of care. The Visit Nurse handles care coordination and collaboration among the Interdisciplinary Group and other medical providers. Assigned patients may be in a home, skilled nursing facility, assisted living facility, or acute care setting.

The Visit Nurse supervises the hospice care provided by Licensed Practical Nurses and Certified Nursing Assistants. Visit Nurses make visits to home patients specific to team nurse coverage needs, unplanned visits, follow-ups on new admissions, or conduct admission visits for medically eligible patients. They also handle facility to home admissions as needed.

The Visit Registered Nurse demonstrates excellence in nursing practice for assignments and supports the organization’s needs in providing nursing care to all Hosparus Health patients. Requirements

  • RN with current RN licensure in the state of Kentucky and/or Indiana
  • BSN desired
  • Certification in hospice/palliative care preferred
  • One year of nursing experience preferred
  • Knowledge of palliative care helpful
  • Valid driver’s license
  • Use of technology-based tools, including electronic medical record, strongly preferred

Responsibilities

  • Assess and identify patient/family needs
  • Plan, implement, evaluate and document individualized hospice nursing care provided to patients
  • Coordinate care and collaborate among the Interdisciplinary Group and other medical providers
  • Supervise hospice care provided by Licensed Practical Nurses and Certified Nursing Assistants
  • Make visits to home patients specific to team nurse coverage needs
  • Conduct unplanned visits and follow-ups on new admissions as needed
  • Perform admission visits for medically eligible patients into Hosparus Health care
  • Conduct facility to home admissions as needed
