About this role
This full-time professional position coordinates nursing and hospice aide staff to ensure day-to-day continuity of care for all Hospice Home patients. It supervises assigned shift Hospice Home RNs, LPNs, and hospice aides, with intermittent oversight of interdisciplinary team members. Accountability aligns with the agency’s mission and regulatory systems.
Supervise, direct, prioritize, and coordinate delivery of patient care on the assigned night shift. Ensure all admissions are assigned appropriately to clinicians and perform weekly chart audits for documentation compliance. Coordinate and problem-solve patient care issues with patients, families, physicians, agency staff, and other providers.
Create a positive environment fostering collaboration and a supportive atmosphere. Demonstrate strong communication, problem-solving, organizational, and delegation skills. Manage personnel actions including hiring, orientation, performance appraisals, promotions, scheduling, counseling, and discipline.
Develop and implement processes to enhance clinical operations and ongoing quality improvement. Oversee medication room operations, regulatory compliance for narcotics, and all clinical accreditation needs. Identify annual competency and staff development needs with PI and Education teams.
Requirements
- RN with current N.C. license
- Two years of previous supervisory experience preferred
- Current CPR certification
- Thorough understanding of Hospice clinical interventions and protocols
- Knowledgeable about reimbursement sources, Medicare/Medicaid regulations, licensing laws and accreditation standards
- Excellent written and verbal communication skills
- Strong team building skills
- Strong leadership abilities
