Provide direct patient care, including assessments and care plan development. Manage a caseload, administer medications and treatments per physician orders, and monitor patient progress while adjusting care plans as needed. Educate patients and caregivers on care and self-management.

Practice independently with autonomy while supported by a collaborative team. Supervise and support HHAs and LPNs, communicate with physicians for continuity of care, and ensure compliance with policies, regulations, and HIPAA standards. Focus on patient-centered care model to spend meaningful time with patients.

Requirements

  • Active RN license
  • Graduate of an accredited nursing program (BSN preferred)
  • Home health or related behavioral health clinical experience
  • Strong communication, organization, and critical thinking skills
  • Current CPR certification
  • Valid driver’s license and reliable transportation

Responsibilities

  • Provide direct patient care, including assessments and care plan development
  • Manage a caseload and coordinate care across the clinical team
  • Administer medications and treatments per physician orders
  • Monitor patient progress and adjust care plans as needed
  • Educate patients and caregivers on care and self-management
  • Communicate with physicians and care teams to ensure continuity of care
  • Supervise and support HHAs and LPNs
  • Complete accurate and timely documentation (OASIS, visit notes, orders)
