Provide direct patient care including assessments and care plan development. Manage a caseload, administer medications and treatments per physician orders, and monitor patient progress while adjusting plans as needed. Educate patients and caregivers on care and self-management.
Communicate with physicians and care teams for continuity of care. Supervise and support HHAs and LPNs while completing accurate documentation like OASIS, visit notes, and orders. Ensure compliance with policies, regulations, and HIPAA standards.

Requirements
- Active RN license
- Graduate of an accredited nursing program (BSN preferred)
- Home health or related behavioral health clinical experience
- Strong communication, organization, and critical thinking skills
- Current CPR certification
- Valid driver’s license and reliable transportation
