About this role
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide comprehensive care for a client in their home. The ideal candidate will have experience in nursing duties and be comfortable providing total care to an individual with complex medical needs such as quadriplegic care, straight catheter, and Hoyer lift use. This role demands attention to detail, following a personalized care plan, and managing daily nursing tasks.
Deliver exceptional direct patient care per established care plans and physician orders safely in the home. Administer medications and treatments, monitor vital signs, and report changes to healthcare professionals. Educate patients and families on procedures while maintaining accurate records per agency policy.

Requirements
- Active LPN license in Florida
- BLS Certification
- A minimum of 2 years related experience
- Proven experience providing nursing care, including tube feeding, straight cath, and the use of a Hoyer lift
- Comfortable with total care responsibilities and maintaining the dignity and privacy of the client
- Strong communication skills and the ability to follow care plans accurately
- Must be able to lift and transfer up to 180 lbs with assistance of Hoyer lift and other assistive devices
- Ability to drive a transport van, with a valid driver’s license and clean driving record
Responsibilities
- Deliver exceptional care to the dedicated client
- Provide direct patient care in accordance with established care plans and physician orders that can be safely provided in the home
- Administer medications and treatments as prescribed by healthcare providers
- Monitor patient vital signs and condition, and report any changes or concerns to appropriate healthcare professionals
- Educate patients and their families on proper healthcare procedures and techniques
- Maintain accurate and up-to-date patient records and documentation according to agency policy
- Demonstrate empathy, compassion, and kindness
- Use sound judgement and remain calm in a variety of situations
