About this role

Signify Health, part of CVS Health, seeks a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. Engage with people in their homes, bridging gaps in care and building trusted relationships. Help make people healthier with flexible, comprehensive assessments delivered right to their doors.

Each visit lasts about 45 minutes for one-on-one connection and answering health questions. Conduct medication and medical history reviews, physical evaluations, and additional diagnostic tests if ordered by the health plan, such as diabetic eye exams or spirometry. Use an iPad and intuitive clinical workflow without prescribing medicine or altering treatments.

As an in-home clinician, work primarily in private residences of health plan members, which may include household pets, secondhand smoke, or airborne allergens. Be flexible to travel locally and within licensed states as needed. Conduct virtual visits and provide all indicated Signify Health services.

Requirements

Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Nurse Practitioner or Physician Assistant
Multi-state licenses (preferred)
Ability to comfortably perform duties in diverse home environments with pets, smoke, or allergens
Flexibility to travel locally and within licensed states
