About this role

CarePartners is one of Ontario’s largest accredited home health care providers, delivering nursing, personal support, therapy and rehabilitation services to approximately 72,000 patients yearly across homes, schools, clinics and retirement homes. We operate 24 locations and 24 nursing clinics, committed to quality and improving patient experiences. We seek RNs and RPNs for Woodstock and surrounding areas in this part-time overnight shift role.

Hours follow a 2-week rotation inclusive of weekends, focused on community health services through our nursing clinics. Nurses develop and maintain effective Care Plans independently or with Ontario Health atHome. In-person patient care includes compression therapy, oncology support, hydration management, IV therapy, ostomy and wound care, plus catheter management.

Requirements

  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Strong desire and commitment to making a difference in the lives of patients
  • Exceptional interpersonal skills
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record

Responsibilities

  • Develop and maintain an effective Care Plan with respect to Community Health Services, independently or in cooperation with Ontario Health atHome
  • Work with patients in-person to provide exceptional nursing services
  • Provide a wide spectrum of services within fully equipped nursing clinic including compression therapy, oncology support, hydration management, IV therapy, ostomy and wound care, catheter management
  • Perform other clinical duties as required
