As a Primary Care Nurse Practitioner, provide prescribed medical treatment and personal care services to patients seeking treatment in the clinic. Collaborate with the core PACT Team including Primary Care Provider, RN, and Medical Assistant, plus expanded team with family, VA, and community services for patient care plans. This is a per-diem position in a clinic open Monday-Friday, 8am-4:30pm.
Actively diagnose and treat veterans under supervising physician direction, examine for disorders, and develop patient management plans. Order and interpret diagnostic tests like x-rays, electrocardiograms, and lab results, then provide follow-up care. Counsel on medications, educate patients, assess mental health, and handle routine health maintenance.
Evaluate records from outside providers using VA co-managed care rules and complete all clinical reminders per visit. Ensure timely documentation within 24 hours and comply with VA guidelines for alerts, consults, and formulary processes. Provide excellent customer service to veterans, colleagues, and participate in outreach events.
Support patient enrollment and retention with scheduling guidance, embrace new clinical or operational initiatives, and focus on excellent outcomes through VA guidelines. Build company culture with patriots in families or as colleagues, committing to world-class patient experience.
Requirements
- Graduate of an accredited school
- Current Nurse Practitioner certification
- Active state Advanced Practice Registered Nurse (APRN) license
- DEA registration for prescriptive authority
- Experience providing primary care in outpatient settings
- Knowledge of VA guidelines and PACT team collaboration
- Ability to interpret diagnostic tests and manage chronic conditions
- Compliance with OIG, Joint Commission and lab standards
Responsibilities
- Actively diagnose and treat veterans under the direction and responsibility of a supervising physician
- Examine patients for symptoms of organic or congenital disorders
- Develop and implement patient management plans and assist in continuity of care
- Order and perform diagnostic tests such as x-ray, electrocardiogram, laboratory tests and interpret results
- Counsel patients on prescription medications, educate patients, assess mental health issues and provide routine health maintenance
- Evaluate patients’ records from medical providers outside the VA for co-managed care
- Complete clinical reminders due at each patient visit and documentation within 24 hours
- Comply with all VA guidelines for patient requests, alerts, consults, orders, lab results and training requirements
